Forever Care Agreement — TenderLovingKittyCare
Parties: [Guardian name] and TenderLovingKittyCare (“Organization”).
Cat: [Name, microchip ID, description].
Purpose: Guardian wishes to ensure lifetime care for the Cat. Guardian transfers legal custody to Organization upon Guardian’s death, incapacity, or immediately if specified below.
Contribution: Guardian agrees to provide a one-time Donation of ……….. as a restricted contribution for the Cat’s lifetime care.
Payment method: _______________ (check / credit / bequest).
Care Provided: Organization agrees to provide lifetime food, litter, routine veterinary care, and emergency medical attention as determined necessary by Organization’s veterinary team. Organization will make reasonable efforts to honor Guardian’s documented care preferences.
Medical Decision Authority: Organization’s medical team will make decisions in the Cat’s best interest in consultation with the designated veterinarian. If major, non-routine procedures are required exceeding the reserved funds, Organization will: (a) use the Cat’s reserve funds; (b) seek supplemental funding.
Refunds/Unspent Funds: If the Cat dies and funds remain, unspent funds will be retained in the Organization’s Forever Care Fund to benefit other enrolled animals unless otherwise specified.
Legal Transfer: Guardian agrees to sign any additional documents required to transfer ownership and provide proof of death or incapacity when applicable.
Amendments: This agreement may be amended only in writing by mutual consent.
